FMLA, physician letter requests, and return to work forms

Please note: when completing FMLA forms, we do not disclose a diagnosis except when we have your consent to do so - the only exception to this being pregnancy.   Fill out these forms to the best of your ability; if you don't know it's okay to tell us you simply don't know.  We're here to help, so if the forms seem cumbersome, we can instead set up a time to go over the forms with you either in an appointment or on the phone with our staff.  Larger forms often require an appointment with your provider.  As such, please instead schedule an appointment for disability forms to be completed in-person.
