Log in to MedXPress and complete your application to the best of your ability before your appointment. If you would like us to review your application with you, save it as a draft and do NOT submit it. Drafts remain saved in the FAA portal for up to 30 days.

Identification: A valid Driver’s License or Government ID (Required).

Vision Correction: Your current eyeglasses and/or contact lenses. If you wear contacts, please bring your lens case and solution, as we must test your vision both with and without correction.

Surgical/Hospital Records: Detailed documentation or clearance letters if you have been hospitalized or had surgery within the last 5 years.
